IT IS THE POLICY OF THE BOARD TO ACCEPT ONLY CASHIER’S CHECKS AND MONEY ORDERS FOR FEES THAT CANNOT BE PAID ONLINE.

All fees collected are non-refundable

  • Application Fee – PT or PTA $100 (online)
  • Temporary License Fee included with Application Fee; no separate or additional fee required
  • Annual Renewal Fee – PT-$130 or PTA- $93 (online)
  • Original License Reprint – $5 (online)
  • Verification of Licensure – no fee
  • Directory of Licensees – $75 per type (PT or PTA), $150 both – no email address included (online)
  • Copy of Records – $1 per page
  • Expired Renewal Fees (after November 1) – PT $230 or PTA $193 and a Restoration Fee of $50; Total of $280 (PT) and $243 (PTA)

A renewal of licensure on or after October 1 is considered late. However, a licensee can renew online up to October 31 if the licensee has not practiced in the State of Alabama. If the licensee has not practiced during the time the license was lapsed, no additional fee will be needed. However, if the licensee practiced with an expired license, additional steps are required. Please call the Board for additional information.

Practicing without a license is a Class C misdemeanor punishable by a fine of not more than $500, or by a term of imprisonment of not more than 90 days, or both. ALA. CODE Section 34-24-196 (1975).
